ADHD takes a toll well into adulthood

Mar. 4, 2013 ? The first large, population-based study to follow children with attention-deficit hyperactivity disorder into adulthood shows that ADHD often doesn't go away and that children with ADHD are more likely to have other psychiatric disorders as adults. They also appear more likely to commit suicide and to be incarcerated as adults. The findings appear in the March 4 online issue of Pediatrics.

"Only 37.5 percent of the children we contacted as adults were free of these really worrisome outcomes," says lead investigator William Barbaresi, M.D., of Boston Children's Hospital, who started the study when he was at Mayo. "That's a sobering statistic that speaks to the need to greatly improve the long-term treatment of children with ADHD and provide a mechanism for treating them as adults."

The study is unique because it followed a large group of ADHD patients from childhood to adulthood, says Slavica Katusic, M.D., an epidemiologist and Mayo Clinic's lead researcher on the study.

ADHD is the most common neuro-developmental disorder of childhood, affecting about 7 percent of all children and three times as many boys as girls. Most prior follow-up studies of ADHD have been small and focused on the severe end of the spectrum -- such as boys referred to pediatric psychiatric treatment facilities -- rather than a cross-section of the ADHD population.

The study followed all children in Rochester who were born between 1976 and 1982, were still in Rochester at age 5 and whose families allowed access to their medical records. That amounted to 5,718 children, including 367 diagnosed with ADHD. Of this group, 232 participated in the follow-up study. About three-quarters received ADHD treatment as children.

At follow-up, the researchers found:

*29 percent of children with ADHD still had ADHD as adults.

*57 percent of children with ADHD had at least one other psychiatric disorder as adults, as compared with 35 percent of those studied who didn't have childhood ADHD. The most common were substance abuse/dependence, antisocial personality disorder, hypomanic episodes, generalized anxiety and major depression.

*Of the children who still had ADHD as adults, 81 percent had at least one other psychiatric disorder, as compared with 47 percent of those who no longer had ADHD and 35 percent of those without childhood ADHD.

*Seven of the 367 children with ADHD (1.9 percent) had died by the time the study began, three of them from suicide. Of the 4,946 children without ADHD whose outcomes could be ascertained, 37 children had died, five by suicide.

*Ten adults who had childhood ADHD (2.7 percent) were incarcerated when the study started.

"We suffer from the misconception that ADHD is just an annoying childhood disorder that's overtreated," Dr. Barbaresi says. "This couldn't be further from the truth. We need to have a chronic disease approach to ADHD as we do for diabetes. The system of care has to be designed for the long haul."

Dr. Barbaresi thinks the study findings may underestimate the bad outcomes of childhood ADHD. Most of those studied were white and middle class, with good educations and access to health care.

"One can argue that this is potentially a best-case scenario," Dr. Barbaresi says. "Outcomes could be worse in socioeconomically challenged populations."

He advises parents of children with ADHD to ensure that their children are in high-quality treatment -- and remain in treatment as they enter adolescence. Children also should be assessed for learning disabilities and monitored for conditions associated with ADHD, including substance use, depression and anxiety.

The study was supported by Public Health Service research grants #s MH076111, HD29745 and AG034676. Pilot work for a portion of the project was funded by an investigator-initiated grant from McNeil Consumer and Specialty Pharmaceuticals.

Why retailers are pinning hopes on Pinterest

NEW YORK -- After a tough day at work as a publicist in Minneapolis, Becca Bijoch would often indulge in a little retail therapy. She usually headed out to the stores as she did not care much for online shopping.

That changed last year when the 26-year-old joined Pinterest, a photo-sharing website that allows users to "pin" images to online bulletin boards based on their interests and to follow others. Bijoch says she has found all sorts of things that she bought after seeing them on Pinterest, from great kitchen tools on CrateandBarrel.com to clothes at Asos.com.

"I'm probably spending more now. I'm on the couch at night, after having two glasses of wine," Bijoch says, but she has no regrets. "I tell everyone that Pinterest has changed my life."

Pinterest, which was the fastest standalone website to hit 10 million unique visitors a month, now has 25 million members, of whom many - like Bijoch - are young, female, well-educated and have disposable income.

Retailers are hankering after these users, but it is sometimes difficult to nab them because Pinterest is an ad-free website and "pins" flourish virally. While many retailers have learned how to interact with consumers on Facebook and Twitter, they are still struggling to figure out Pinterest and the ways to make money out of reaching shoppers through it.

In that effort, many retailers have installed Pinterest buttons on their main websites, created their own Pinterest pages, and allocated marketing dollars to acquire followers. While Pinterest says it does not track metrics internally, many ancillary businesses have popped up to help companies harness the revenue-driving possibilities of the site.

"It's a huge window-shopping platform," says Kyla Brennan, chief executive of HelloInsights, a Santa Monica, California company that provides analyses of Pinterest use. "It helps people find what they really like. Does it encourage people to be a little impulsive? Of course."

E-commerce experts say Pinterest generates more dollars per users than some other social media sites, even though Facebook, the world's largest social network with more than a billion users, is a leading driver of shopping by volume.

Pinterest shoppers, on average, spend nearly $170 per session, according to a study by RichRelevance, an e-commerce consultant, which tracked 700 million shopping sessions. In comparison, Facebook shoppers spend $95 per session, while Twitter shoppers spend $70.

Major retail brands with a presence on Pinterest include L.L. Bean, with more than 5 million followers; Nordstrom Inc., with more than 4 million followers; and Lululemon Athletica Inc. with close to 2 million. Other major players like Gap Inc. and Urban Outfitters Inc. have fewer followers on the site, but are growing.

The three-year-old Pinterest recently closed a $200 million round of financing, which raises its value to $2.5 billion.

Pinning costs
Most companies' Pinterest activity is handled by their social media teams, but the efforts differ from promotions on Facebook and Twitter because Pinterest boards are interest-based and not timeline-based. Nordstrom's Facebook page, for example, features a couple of products daily that are on sale or seasonally interesting, but its Pinterest page is an evergreen collage of fashion lifestyle images.

As there is no direct advertising and Pinterest is still growing, the marketing cost to businesses of acquiring new users is lower than other sites, says Daniel Maloney, CEO of PinLeague, a consultancy that tracks social media usage.

"The current cost of acquiring a Pinterest follower is a penny to 50 cents, depending on type of business. That compares to 50 cents to $2.50 on Facebook," he said.

One of the top ways to reach followers seems to be to do anything but try to sell products. L.L. Bean's most popular board, for instance, is devoted to pictures of woodland creatures - its most popular pin is a picture of a cat dressed up to look like a bat.

"While we do measure traffic coming to llbean.com and llbeansignature.com from Pinterest, we are not currently promoting the platform as a selling channel," says Laurie Brooks, senior public relations representative for the company based in Freeport, Maine. Internal metrics show Pinterest users spend more than others, and that traffic is on the increase even if, overall, it is less than what comes from Facebook, she adds.

L.L. Bean, like many other retailers, has not offered discounts through Pinterest, but it has sponsored contests. So has Nordstrom, which did a bridal-focused sweepstakes in January.

"Our followers share and save compelling imagery and merchandise - period. Our boards aren't focused on bargains or coupons," says Bryan Galipeau, who is group manager of social media at Nordstrom's Seattle headquarters. "We think it's important to take a broader view of Pinterest because that's also how our customers see it."

Fashion retailer Gilt.com's "Pin to Win" contest offered a $2,500 shopping spree to women who shared certain images from their wedding dress collection. Gilt also rewarded those who received 50 re-pins of a children's dress the chance to buy the item for 77 percent off.

Similarly, fashion-seller Karmaloop.com offered a trip to Paris for the best outfit pinned to one of its boards.

Other goodies from retailers include AMC Theatres' giveaway pinboard where users can win movies posters and other memorabilia. Wal-Mart Stores Inc. ran a contest about inspiring people to be environmentally conscious.

Even if you could win something by shopping through Pinterest, consumers need to exert some impulse control.

Since most of what is "pinned" on the site is aspirational, people can end up spending way more than when they click through from other social media sites.

T?ib?n?s talk is under the auspieces of Bard?s annual Eugene Meyer Lecture in British History and Literature. This dedicated annual lecture was established in 2011 to commemorate Eugene Meyer (1875?1959), the owner and publisher of the Washington Post, chairman of the Federal Reserve, and first president of the World Bank. The lecture is presented in association with the endowment of the Eugene Meyer Chair in British History and Literature at Bard College. Professor Richard Aldous holds the Eugene Meyer Chair.

Colm T?ib?n was born in Enniscorthy, Co. Wexford, in 1955. He studied at University College Dublin and lived in Barcelona between 1975 and 1978. Out of his experience in Barcelona be produced two books, the novel The South (shortlisted for the Whitbread First Novel Award and winner of the Irish Times/Aer Lingus First Fiction Award) and Homage to Barcelona, both published in 1990.

When he returned to Ireland in 1978 he worked as a journalist for In Dublin, Hibernia and The Sunday Tribune, becoming features editor of In Dublin in 1981 and editor of Magill, Ireland?s current affairs magazine, in 1982. He left Magill in 1985 and travelled in Africa and South America. His journalism from the 1980s was collected in The Trial of the Generals (1990). His other work as a journalist and travel writer includes Bad Blood: A Walk Along the Irish Border (1987) and The Sign of the Cross: Travels in Catholic Europe (1994).

His other novels are: The Heather Blazing (1992, winner of the Encore Award); The Story of the Night (1996, winner of the Ferro-Grumley Prize); The Blackwater Lightship (1999, shortlisted for the IMPAC Dublin Prize and the Booker Prize and made into a film starring Angela Lansbury); The Master (2004, winner of the Dublin IMPAC Prize; the Prix du Meilleur Livre; the LA Times Novel of the Year; and shortlisted for the Booker Prize); Brooklyn (2009, winner of the Costa Novel of the Year).

His short story collections are Mothers and Sons (2006, winner of the Edge Hill Prize) and The Empty Family (2010, shortlisted for the Frank O?Connor Prize). His play Beauty in a Broken Place was performed at the Peacock Theatre in Dublin in 2004. His other books include: The Modern Library: the 200 Best Novels Since 1950 (with Carmen Callil); Lady Gregory?s Toothbrush (2002); Love in a Dark Time: Gay Lives from Wilde to Almodovar (2002) and All a Novelist Needs: Essays on Henry James (2010).

He has edited The Penguin Book of Irish Fiction. His work has been translated into 30 languages. In 2008, a book of essays on his work Reading Colm T?ib?n, edited by Paul Delaney, was published.

He has received honorary doctorates from the University of Ulster and from University College Dublin. He is a regular contributor to the Dublin Review, New York Review of Books, and London Review of Books. In 2006 he was appointed to the Arts Council in Ireland.

He has twice been Stein Visiting Writer at Stanford University and has also been a visiting writer at the Michener Center at the University of Texas at Austin. He also taught at Princeton between 2009 and 2011, and was professor of creative writing at the University of Manchester in the autumn of 2011. He is currently Mellon Professor in the Department of English and Comparative Literature at Columbia University.

In 2011 his play Testament, directed by Garry Hynes, was performed in the Dublin Theatre Festival with Marie Mullen in the lead role. Also in 2011, his memoir A Guest at the Feast was published by Penguin UK as a Kindle original. In 2012 his new collection of essays, New Ways to Kill Your Mother: Writers & Their Families was published.

Fintan O?Toole, the Leonard L. Milberg ?53 Visiting Lecturer in Irish Letters at Princeton for Spring 2012, is one of Ireland?s leading public intellectuals. He has served as a drama critic for The Irish Times, New York Daily News, Sunday Tribune (Dublin), and In Dublin magazine. His books on theater span a wide range of topics, from his biography of Richard Brinsley Sheridan to whatever is now appearing on Irish stages. He is currently assistant editor, columnist and feature writer for The Irish Times. He also contributes to The New York Review of Books, The New Yorker, Granta, The Guardian, The Observer, and other international publications. In 2011, O?Toole was named one of ?Britain?s top 300 intellectuals? by The Observer. He has received the A.T. Cross Award for Supreme Contribution to Irish Journalism, the Millennium Social Inclusion Award, and Journalist of the Year in 2010 from TV3 Media Awards.

Eugene Meyer (1875?1959) was an American financier, influential leader in American political and social life, and publisher of the Washington Post from 1933 to 1946. After graduating from Yale University in 1895, he worked at Lazard Freres banking house in New York, and in 1901 bought a seat on the New York Stock Exchange. By 1917 he had made a personal fortune by focusing on investments in the copper, gold, automobile, and chemical industries. He developed a reputation on Wall Street as a solid manager of investment funds and as an innovator. His firm pioneered the use of a research department?scientific investing?to perform detailed economic analyses of businesses. He had a great sense of public duty, and served at the Versailles Peace Conference, on the Federal Farm Loan Board, at the Reconstruction Finance Corporation, and, from 1930, as chairman of the Federal Reserve. In 1933, Meyer left the political world and purchased the Washington Post. He returned to public service during World War II to serve on the National Defense Mediation Board, and after the war planned to spend the rest of his career at the Washington Post. However, in 1946 President Truman asked him to head the World Bank, and Meyer accepted. He held the post for six months, establishing the World Bank, before returning to his work at the Washington Post until his death in 1959.

NEW YORK (AP) ? U.S. stock futures are following a sell-off in China after it put into place new measures to slow a white-hot housing market.

Dow Jones industrial futures fell 20 points to 14,054 Monday. The broader S&P futures have lost 1.7 points to 1,514.80. Nasdaq futures are down 7 points to 2,741.75.

China announced taxes on housing profits that will weigh on overall housing prices.

The U.S. is already seeing major industries affected by the debt crisis in Europe and a cooling economy in China, the world's second largest economy, would be felt here.

It is a light week for economic indicators until Friday when the Commerce Department releases its jobs report for February. Employers have added an average of 200,000 jobs a month from November through January.

The past decade has now seen at least three industry-wide technologies vie for the future of television -- HD, 3D and now 4K or UHD. The first of these -- HD -- represented a massive change for television that affected nearly every aspect of the TV experience from how it was captured to how it was consumed. A decade later, it is nearly impossible to purchase a TV that does not support high-definition. The second -- 3D -- was a mixed bag. While the technology became commonplace on high-end TVs, it has remained relevant for only a small fraction of programming. The question, then, is which of these paths, if either, 4K will follow.

So why did March 4 become the real deadline for the Ravens to get a deal done with quarterback Joe Flacco?

Because the Ravens didn?t want to have to choose between the non-exclusive and exclusive level of the franchise tag.

The latter, in light of the recent restructurings by Tom Brady and Ben Roethlisberger, would have put Flacco on track to make more than $74 million over three years, if Flacco opted to take it year to year under the franchise tag.? (That?s more than $24 million per year.)? The former would have opened the door for a front-loaded monster offer from another team that the Ravens couldn?t have matched without dumping plenty of players.

That was enough to persuade the Ravens to get it done before encountering a fork in the road with a pair of paths that led nowhere good.

Unless the Falcons and quarterback Matt Ryan work out a new deal within the next 12 months, Atlanta G.M. Thomas Dimitroff will face a similar dilemma.

Per a source with knowledge of Ryan?s contract, his cap number in 2013, the final year of his rookie contract, will be $12 million.? Which means that his non-exclusive tender would be no higher than the base quarterback amount for 2014, since a 20-percent raise over $12 million will be only $14.4 million.

The exclusive number will be several million higher, setting the stage for a much higher three-year haul under a formula that would give Ryan a 20-percent raise for the second year of the tag and a 44-percent raise in the third.

The difference in this case is that Ryan is represented by CAA, which handles an ever-growing stream of franchise and highly-paid quarterbacks (Eli and Peyton Manning, Drew Brees, Matthew Stafford, Tony Romo, Sam Bradford, Philip Rivers, Robert Griffin III) and which consistently emerges from these talks with top-of-market deals, team cap situation be damned.? Now that the contract CAA negotiated in 2012 for Drew Brees has been eclipsed by the non-CAA Flacco deal, the goal will be to get back on top.

It?ll likely happen with Ryan, whether now or later or on the brink of the franchise-tag dilemma.? Or at some point after that.
